Bidding Requirements and Information.

- All Questions/Clarifications through the Forum deadline will be January 5, 2017. (So please ask them ASAP)
- If Phone conversations are initiated, then ALL Questions MUST be created on this forum (In respective Locations) and will be answered for "Fair Bidding" Processes. NO EXCEPTIONS!
- Bids are due by January 12th, 2017 @ 4:00PM, either by Snail Mail and/or E-mail (Preferred).
- All Requested documents/references to be included in your Bid:
a. RFP Number: 470-2017-2018-JEFFERSON-CAT1-#170052315
b. USAC SPIN#
c. Company information, background and experience, and contact information (may include sales, service, E-Rate and accounting with phone numbers & emails).
- Plus, a complete copy of the contract terms.
d. Statement of Work / Services including maintenance & warranty information.
e. List of References (minimum 3 – within requested mileage requirement)
f. Copy of the list of posted questions & answers – as posted on JISD's E-Rate Q&A Forum
g. Federal Tax ID certificate (W-9)
